Stages of Cervical Cancer

Stage I

Carcinoma strictly confined to the cervix (extension to uterus not considered).

  • IA: Invasive carcinoma diagnosed only by microscopy. Stromal invasion ≤5 mm.
  • IA1: Stromal invasion ≤3 mm.
  • IA2: Stromal invasion >3 mm and ≤5 mm.
  • IB: Stromal invasion >5 mm, limited to cervix.
  • IB1: Tumor ≤2 cm.
  • IB2: Tumor >2 cm and ≤4 cm.
  • IB3: Tumor >4 cm.

Stage II

Carcinoma extends beyond cervix but not to pelvic wall or lower third of vagina.

  • IIA: Without parametrial invasion.
  • IIA1: Tumor ≤4 cm.
  • IIA2: Tumor >4 cm.
  • IIB: With parametrial invasion, not to pelvic wall.

Stage III

Tumor involves lower third of vagina, pelvic wall, or lymph nodes.

  • IIIA: Lower third of vagina, no pelvic wall extension.
  • IIIB: Pelvic wall extension or hydronephrosis/non-functioning kidney.
  • IIIC: Pelvic/para-aortic lymph node involvement.
  • IIIC1: Pelvic lymph nodes.
  • IIIC2: Para-aortic lymph nodes.

Stage IV

Carcinoma extends beyond true pelvis or involves bladder/rectum mucosa.

  • IVA: Spread to adjacent pelvic organs (bladder/rectum), confirmed by biopsy.
  • IVB: Distant metastasis (e.g., lungs, liver, bone).
